Maca Root for Women: Energy, Libido and the Hormone Myth
Maca is sold almost everywhere as a hormone balancer. It appears in perimenopause blends, libido stacks and energy powders, usually under a headline about "rebalancing your hormones."
The clinical literature says something considerably more interesting: in the trials where researchers actually measured hormones, maca didn't change them — and women still reported feeling better.
That's not a weaker story. It's a different mechanism, and understanding it tells you a lot about what maca is good for and what it isn't.

What maca is
Maca grows in some of the least hospitable farmland on earth: the Junín plateau in the central Peruvian Andes, above 4,000 metres, where frost, UV exposure and thin air kill most crops. It has been cultivated there for at least two thousand years, eaten as food and used as a traditional remedy for stamina, fertility and altitude endurance.
It comes in colours — yellow, red and black — determined by the root's pigmentation. Yellow is the most common and the most studied; red and black have preliminary research suggesting different emphases, but the comparative evidence is too thin to make confident claims about.
Maca's characteristic compounds are macamides and macaenes, fatty acid derivatives not found in other plants, along with glucosinolates shared with its brassica relatives. These are the compounds most often proposed as the active fraction, though the mechanism remains less well characterised than, say, ashwagandha's.
The hormone myth, and what's actually going on
In 2008, Brooks and colleagues published a randomised, double-blind, placebo-controlled crossover trial in Menopause. Postmenopausal women took maca; researchers measured psychological symptoms, sexual dysfunction, and hormone levels.
Symptoms improved. Hormones didn't move. The paper's title says it outright: "Beneficial effects of Lepidium meyenii (Maca) on psychological symptoms and measures of sexual dysfunction in postmenopausal women are not related to estrogen or androgen content."
A separate crossover trial gave 14 postmenopausal women 3.5 g/day of powdered maca for six weeks against matched placebo, and found no differences in serum estradiol, FSH, LH or SHBG between baseline, maca and placebo.
So maca is not topping up your estrogen. The prevailing hypothesis is that it acts as an adaptogen at the level of the hypothalamic–pituitary axis — influencing signalling and stress adaptation rather than adding or mimicking a hormone.
Which means: if you've been told maca will "balance your hormones," the honest correction is that it appears to help some women feel better without balancing anything measurable. The claim is wrong; the herb may still be useful.
What the research supports
Sexual function and libido
This is maca's best-evidenced territory.
A double-blind placebo-controlled trial published in CNS Neuroscience & Therapeutics tested maca root in 42 women — 30 premenopausal, 12 postmenopausal — experiencing antidepressant- induced sexual dysfunction. Remission rates were higher on maca than placebo across multiple sexual function measures, and the effect was strongest in the postmenopausal participants.
The 2008 Menopause trial above found improvements in measures of sexual dysfunction alongside its psychological findings.
This matters because sexual side effects of SSRIs and the libido changes of the menopause transition are both routinely under-discussed, and options that don't involve hormones are limited.
Menopausal symptoms
A 2011 systematic review in Maturitas by Lee and colleagues searched 17 databases and found four randomised controlled trials of maca for menopausal symptoms in peri-, early post- and late postmenopausal women. Using the Kupperman Menopausal Index and the Greene Climacteric Scale, all four reported favourable effects versus placebo.
The review's own conclusion was appropriately restrained: the total number of trials was small, sample sizes were modest, and methodological quality varied. Fifteen years later, the evidence base has grown, but not transformed.
Energy and mood
Reports of improved energy and mood appear throughout the trials as secondary outcomes, consistent with the adaptogen framing. This is the least rigorously isolated of maca's claimed effects — it tends to ride along with the primary endpoints rather than being tested head-on.
Dosing: the number that trips everyone up
Here's where most maca marketing becomes misleading, and it's worth understanding before you compare any two products.
Clinical trials used raw powdered maca root at 1.5 g to 3.5 g per day. Those are grams — a teaspoon or more of powder, which is why maca is traditionally sold as a powder you stir into things.
Concentrated extracts are a different unit entirely. A 10:1 extract means ten parts root produced one part extract. So 200 mg of a 10:1 extract corresponds to roughly 2 g of root — inside the studied range, but only when you state it as an equivalence.
A product listing "200 mg maca" without specifying the extract ratio is either dramatically under the studied dose or using a concentrate and not telling you. This single unstated variable is the most common way maca products mislead by omission.
Safety
Maca is a food crop with a long dietary history and a good safety record in trials. Reported side effects are mild and uncommon — digestive upset, occasionally jitteriness or disturbed sleep if taken late in the day.
Points of caution:
- Thyroid conditions — maca is a brassica, and brassicas contain glucosinolates, which in large quantities may affect iodine uptake. Cooked or gelatinised maca reduces this. Worth discussing with your doctor if you have thyroid issues.
- Pregnancy and breastfeeding — insufficient safety data; avoid.
- Hormone-sensitive conditions — maca doesn't appear to act hormonally, but individual medical advice still applies.
